Overview of GDM

GDM is characterized by the inability of pancreatic β-cells to respond adequately to the increased insulin requirements of pregnancy resulting in varying degrees of hyperglycemia. Many risk factors for developing GDM are similar to those for type 2 diabetes, including obesity, family history of diabetes, and high-risk ethnicities. Additional risk factors include increased maternal age, fetal macrosomia, and personal history of GDM.

Therapies of GDM

Gestational diabetes is treated with a diabetic diet, exercise, medication (such as metformin), and sometimes insulin therapy. Management of GDM should occur in a multidisciplinary setting with joint diabetes and obstetric input. Most women can manage their GDM with dietary changes and exercise.

  • Pharmacological Therapy
    If blood glucose targets are not achieved with lifestyle intervention, the addition of glucose-lowering therapy is warranted. The pharmacological therapy mainly includes multiple daily insulin injections, oral metformin and glyburide (the only oral agents used in the treatment of GDM), and some safe insulin analogs (rapid and long acting) during pregnancy.
